2012 AzPHA Member Survey

AzPHA is seeking your input on the benefits of AzPHA Membership. Your feedback is very valuable to us and your participation in our survey will help us to improve and enhance AzPHA. All information is confidential and your participation in the survey is voluntary.

We kindly request that you set aside 15-20 minutes to complete the survey. To complete the survey, please visit the member only section to participate.

The “2012 AzPHA Member Survey ” will close on Friday, August 31, 2012, 11:59 pm EST. Survey data will be presented at the Annual Meeting on Thursday, September 20, 2012. If you have any questions about the survey or if you experience difficulties, please feel free to contact the office.

EVENT REFUND POLICY:Event registration fees can be refunded with written notice up to two weeks prior to the conference date. Deadlines for refund requests will be printed in event materials. After the deadline, no refunds will be given. Memberships and donation that have been charged are non-refundable.
