Simply click on “Membership” and fill out the form. If you are an employee of one of our Sustaining Members, you can get a 15-20% discount on your annual membership fee. Click on the menu at the left to see a list of our Sustaining Members.

Current Members Information

Current members can locate information about advocacy, meeting locations, dates and times and other general information about the organization. Past minutes of meetings, and past agendas are available in the Members Only section. Please refer to the AZPHANET list serve for the password to gain access to the Member Only section!! The list serve is available for members at the time they sign up and all requests should go to the Executive Director, Susan Higgins.

Refund policy statement:

Conference registration fees can be refunded with written notice up to two weeks prior to the conference date. Deadlines for refund requests will be printed in all conference brochures. After the deadline, no refunds will be given. Memberships and donations that have been charged are non-refundable.
