Communication in Serious Illness

This Free 2.5 hour didactic sessions objectives will review the need for a systematic approach to having more, better and earlier conversations about patient values and priorities in serious illness, define a population with serious illness who may benefit from the serious illness conversation,  and learn how to improve communication in patients with serious illness with a structured communication tool.  During this highly interactive workshop participants will have the opportunity to practice case study conversations.  Target audience is healthcare teams such as physicians, nurses, social workers and other healthcare providers.
